The information below is provided in accordance with Texas House Bill 4224 (effective September 1, 2025) and Section 181.105 of the Texas Health and Safety Code. While most concerns between a client and clinician can be resolved through direct communication, some situations may require formal escalation. In compliance with the State of Texas, the information below is provided to assist you in requesting your records, contacting the Texas Behavioral Health Executive Council, and submitting a consumer complaint if needed.
Business Violation Concerns
If you believe your rights have been violated, you may file a complaint with the Texas Office of the Attorney General.

The Texas Behavioral Health Executive Council oversees the following licensing boards:

• Texas State Board of Examiners of Professional Counselors
• Texas State Board of Examiners of Psychologists
• Texas State Board of Social Worker Examiners

Request Records
You have the right to request a copy of your mental health records.
SanaMente Wellness, PLLC is required to retain client records for seven (7) years, or five (5) years after a former minor client reaches the age of majority whichever is greater, in accordance with Texas record-retention requirements.
To request your records from SanaMente Wellness, PLLC, please:
Submit a written request via your client portal or by email.
Include your full name, date of birth, and the specific records you are requesting.
Allow up to 14 business days for processing.

Good Faith Estimate
Under the "No Surprises Act," as of January 1, 2022 medical facilities are required to provide a "Good Faith Estimate" to patients not planning to use insurance. The Good Faith Estimate is not a contract and doesn’t obligate you to receive services. It’s provided to help you understand the expected costs of services. Actual charges may vary depending on the services you choose, changes in treatment frequency or duration, and other clinical factors. You have the right to decline services at any time. If your session frequency increases/decreases or additional services are added, you’ll receive an updated Good Faith Estimate reflecting the revised scope and anticipated cost of care.
